LeadingCare Network Texas is a network exclusively dedicated to serving and representing the needs of Texas non-profit, post-acute, long-term care providers.

Members have access to resources, contract review and negotiating services, utilization reports and scoring that shows how their facilities rank both statewide and nationally.

Plus, members have a dedicated managed care team for contract negotiation, credentialing services, and a robust quality program.

OUR MISSION

To maximize the health and well-being of seniors in its member communities through innovative, cost-effective care management practices and quality improvement activities through contracting relationships with health plans and health delivery systems.

Grant Swemba, MBA

Senior Vice President, Managed Care

As Senior Vice President of Managed Care at Strategic Health Care, Grant brings over 25 years of comprehensive post-acute care knowledge across multiple service domains including skilled nursing, assisted living, home health, hospice, outpatient rehabilitation, and adult day care. His financial and operational contract development expertise is focused on building sustainability and ensuring that mandates are met. He offers essential knowledge vital to the development of effective alternative payment initiatives, including value-based reimbursement and pay for performance models focused on quality outcomes and reduced spending.

Carly Draeger

Vice President of Contracting & Provider Support

In her role as Vice President of Contracting and Provider Support, Carly brings deep industry knowledge, leadership experience, and a strong background in provider negotiations and network strategy.

Carly joined Strategic Health Care as Manager of Contracting and Process Development, bringing more than ten years of experience in the managed care industry. She began her career in 2012 as a Contract Coordinator with a northwest Ohio health insurance company, supporting agreement negotiations. Within two years, she advanced to Provider Contract Specialist, negotiating physician and ancillary agreements.

In 2016, Carly expanded her expertise into Medicare marketing compliance for managed care plans. She returned to Network Management in 2017 as Senior Provider Contract Specialist, where she negotiated physician, ancillary, value-based, and behavioral health agreements while mentoring other contract specialists.

In 2019, Carly accepted a Network Contract Manager position with a national health insurance company. In that role, she focused on physician agreements, including value-based arrangements, and supported overall provider network management.

Arlene Jaroscak, RN, LNHA

Senior Vice President of Network Management & Quality

As Senior Vice President of Network Management and Quality at Strategic Health Care, Arlene works with health care networks, managing their Quality and Contracting efforts, as well as the pay for performance initiatives of providers, including facilities, physicians and suppliers.  Arlene’s visionary executive leadership in healthcare and her passion for quality and serving seniors, has traversed the long-term care continuum.  She is a Licensed Nursing Home Administrator, Registered Nurse, Clinical Consultant, Conference Presenter, Speaker, and Author. Arlene received her Bachelor’s Degree in Long-term Care Administration from St. Joseph’s College of Maine and is a member of the Leadership Lorain County Class of 2009.
