LeadingCare Network Texas is a network exclusively dedicated to serving and representing the needs of Texas non-profit, post-acute, long-term care providers.
Members have access to resources, contract review and negotiating services, utilization reports and scoring that shows how their facilities rank both statewide and nationally.
Plus, members have a dedicated managed care team for contract negotiation, credentialing services, and a robust quality program.
OUR MISSION
To maximize the health and well-being of seniors in its member communities through innovative, cost-effective care management practices and quality improvement activities through contracting relationships with health plans and health delivery systems.
Meet The Team

Celeste Mackey, LNHA, MHA
Vice President of Quality
Celeste is a health care professional with over 24 years of experience in the continuum care environment, including community-based services, independent and assisted living, rehab, and long-term care services. She has extensive experience in revenue cycle management, including insurance reimbursement and contract analysis, and strong acumen in the clinical financial relationship with an emphasis on quality improvement. Celeste is currently pursuing a Doctoral Degree in Healthcare Administration with a concentration in healthcare policy. Celeste’s Christian beliefs support her dedication to her family, and her passion for health care. She enjoys traveling, spending time in nature, and shopping. Maintaining a good work life balance is a priority for Celeste and supports her personal and professional values.

Grant Swemba, MBA
Senior Vice President, Managed Care
As Senior Vice President of Managed Care at Strategic Health Care, Grant brings over 25 years of comprehensive post-acute care knowledge across multiple service domains including skilled nursing, assisted living, home health, hospice, outpatient rehabilitation, and adult day care. His financial and operational contract development expertise is focused on building sustainability and ensuring that mandates are met. He offers essential knowledge vital to the development of effective alternative payment initiatives, including value-based reimbursement and pay for performance models focused on quality outcomes and reduced spending.

Carly Draeger
Manager of Contracting & Process Development
Carly comes to Strategic Health Care as the Manager of Contracting & Process Development with over ten years of experience in the managed care industry. Carly quickly launched her career in 2012 as a Contract Coordinator for a local northwest Ohio health insurance company, where she assisted in negotiating agreements. After only two years she was promoted to a Provider Contract Specialist. Carly negotiated physician and ancillary agreements. In 2016, Carly gained experience in Medicare marketing compliance regulations for managed care plans. In 2017 Carly returned to Network Management as the Senior Provider Contract Specialist where she negotiated physician, ancillary, value based, and behavior health agreements while also mentoring the Provider Contract Specialists. In 2019, Carly’s career continued to grow and develop as she accepted the Network Contract Manager role at a national health insurance company. There she specialized in physician agreements including value based and managing the provider network.
